New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

 New patients will be encouraged to see our practice nurse soon after registering so that we can familiarise ourselves with your medical history and treatment. Medical records can take a considerable time to reach us and this examination gives us valuable information about you until they arrive.

If you move please let us know your new address and telephone number. If you move outside our practice area, please do not assume that we can continue to care for you.
